For field complaints or if you suspect you may have defective parts, please complete the following steps:
- Please complete the Field Complaints form (click here to download)
- If you have 1-3 defective items you may send them to:

Make sure to include the following:
- Proof of purchase
- Completed "Field Complaints" form
- If you have 4 or more defective parts, an RGA must be assigned
To have an RGA assigned, complete the "Field Complaints" form and attach a copy of the proof of purchase and send the completed "Field Complaints" form and proof of purchase to via fax to: 773-804-0180 or email to email@example.com
- A customer service representative will contact you as soon as possible. If parts are returned someone will contact you after the Radionic Engineering Team completes its diagnostic process. For faster service, please be sure to complete the entire form with as many details as possible.
